Hills Medical Center. Job Summary and Qualifications The
Stroke/Sepsis Coordinator for TriStar Southern Hills Medical
Center, with the assistance of the Stroke Team Medical Director,
Director of Emergency Services, Director of Critical Care, and
Director of Quality and Risk Management, is responsible for
assuring a coordinated effort in fulfilling department
responsibilities with the long-range plan, mission, vision, and
values of being an Accredited Stroke Center. The Coordinator is
responsible for ensuring the department’s participation in meeting
the goals and objectives for Performance Improvement activities to
assure Quality Improvement. The Coordinator provides leadership and
administrative functions, ensures that patient care is based on the
Nursing Process, and evaluates the effectiveness on an on-going
basis. This role is responsible for establishing departmental
policies and procedures to ensure effective, efficient, and
continuous quality work for the Stroke Center processes. What you
will do in this role: - Coordinate stroke program with the
multidisciplinary team for optimal patient outcomes. - Provide
oversight for Joint Commission disease-specific certification for
primary and or/comprehensive stroke center. - Provide leadership
and educational support for the clinical staff on stroke units. -
Make daily rounds to ensure compliance with core measures and
evidence-based care. - Work collaboratively with EMS and
post-hospital partners to improve stroke protocols and programs. -
Coordinate care for the neurological patient across the continuum,
including stroke screening, education, follow up phone calls,
support groups, and community outreach. What qualifications you
will need: - Graduate of an accredited School of Professional
Nursing required. - Bachelor of Science in Nursing required;
Master’s Degree preferred. - Current licensure as a Registered
Nurse (RN) in the state of Tennessee or compact state required. -
Current BLS, ACLS, NIHSS, CCRN or CNRN preferred. - 3 years of
experience in acute care required; neurological nursing experience
