Job Description Job Description Hospice RN Case Manager Job Summary: The Hospice RN Case Manager is responsible for providing compassionate, comprehensive end-of-life care to patients in their homes or care facilities. This role involves coordinating care plans, managing symptoms, and supporting patients and families through the hospice journey. Key Responsibilities: Coordinate and manage care for hospice patients in collaboration with the interdisciplinary team. Perform initial and ongoing assessments of patients’ physical, emotional, and psychosocial needs. Develop and implement individualized care plans focused on comfort and quality of life. Administer medications and treatments to manage symptoms effectively. Educate patients and families on disease progression, symptom management, and hospice philosophy. Serve as a liaison between patients, families, physicians, and other care providers. Participate in on-call rotation for after-hours support. Document care accurately and timely in accordance with regulatory standards. Qualifications: Current RN license in TN (unencumbered). Minimum of 1 year of nursing experience (hospice or palliative care preferred). Valid driver's license, reliable transportation, and proof of insurance. Strong communication, organizational, and critical-thinking skills. Ability to work independently and manage a caseload effectively. Preferred: Experience with EMR systems and interdisciplinary team collaboration Experience with Hospice and End of Life Care
