You will have the satisfaction of knowing you're ensuring the patient receives optimal care, by assessing, planning, implementing, reevaluating and revising care and coordinating resources. Report significant changes immediatelyObserve, assess and record patient's physical and mental condition and reports any changesAdminister medications as instructed and as ordered by the physician and instructed by the RN. Document the patient's file accordinglyAttend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assignedCooperate with on-call schedule, weekends and after hour visits, as necessary Requirements Licensed Practical Nurse / LPN license in Tennessee in good standingPrevious experience in home health, hospice, hospital or skilled nursing preferredFamiliarity with medical software and equipment - Brightree a plusAbility to build rapport with patientsStrong problem solving and critical thinking skillsAbility to thrive in a fast-paced environmentReliable transportation, valid driver's license and current automobile insurance
