Wedgewood Healthcare Center, a member of the CommuniCare Family of Healthcare Companies, is currently recruiting an experienced health care professional, with experience in Dementia and/or Psychiatric care preferred, to manage programming on our Memory Care Unit.We are looking for someone who can apply a creative and holistic approach to care for the behavioral complexities of those living with dementia.PURPOSE/BELIEF STATEMENTThe Memory Care Program Director is responsible for the delivery of efficient and effective dementia care while achieving positive clinical outcomes and resident and family satisfaction. While operating within his/her scope of practice, the Memory Care Program Director delegates aspects of resident care to licensed and unlicensed staff consistent with their scope of practice and in accordance with CommuniCare's Philosophy and Standards of Care. The position requires that the individual develoip and maintain a quality program that therapeutically addresses the needs of residents with dementia and their families.JOB RESPONSIBILITIESThe Memory Care Program Director's role will include:

  • Use a validation approach to communicate with those living with a dementia diagnosis.
  • Effectively facilitate group activities.

    • No license/certifications required, but previous work with dementia care preferred.
    • One (1) to two (2) years management/supervisory experience preferred.KNOWLEDGE/SKILLS/ABILITIES
      • Thorough understanding of: Operational services terminology, principles and practices of dementia care * Person-centered care philosophies and principles * Medicare and Medicaid guidelines * Health care principles, practices, regulations, concepts and procedures as applied to the care of patients in a skilled or assisted living facility * Compliance with external regulatory agencies and organizations for all areas of responsibility
      • Ability to communicate effectively, both orally and in writing, and identify and recommend solutions to problems
      • Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
      • Ability to review and analyze report information while using critical thinking skills to evaluate data for regulatory and standards of compliance
      • Highest level of professionalism with the ability to maintain confidentiality.
      • Ability to work independently, as well as collaboratively, within a team environment
      • Ability to provide a high level of customer service in a calm and professional manner
      • Ability to deal tactfully with personnel, campus department leaders, residents, family members, visitors, government agencies/personnel, vendors, and the general public
