Develops a holistic multidisciplinary plan of care utilizing the job functions within each of the five core components of case management. Interacts with the care management team and physicians to determine appropriate Homecare service for patients/clients based on regulatory guidelines for each line of service. Resource to case managers for wound assessments and manages/follows up on declines in wounds of homecare patients. Reviews changes in the plan of care, medication profiles etc to facilitate timely discharge to appropriate Homecare service line. Collaborates with other health professionals and paraprofessionals when necessary. Provides clinical supervision of the multi-disciplinaryteam. Instructs and educates staff, client, clients family and volunteers regarding provision of care. Participates and assists with Lourdes Homecare/ Hospiceorientation and in-service training programs. Participates in organizational committees. Utilizes appropriate care standards to coordinate client care withdocumentation to support variances as they occur. Individualizes plan of care within care standard to effect maximal client outcomes. Implementsdischarge planning upon admission and coordinates discharge throughout care. Consistently assesses client needs using appropriate integrates tools.Coordinates client conference to enhance full team discussion consultation and evaluation of plan of care, Assist other case managers, share in providing 24-7 coverage and accepts other assignments as appropriate, Demonstrates working knowledge and adheres to agency policy and procedures and to state andfederal governing regulations.Department Specific Functions (2-4 statements)Complies with and is knowledgeable of hospital and department safety procedures and policies. Contributes to the care of environment by maintaining work areaand assisting with light housekeeping duties as needed. Is knowledgeable and participates in hospitals Performance improvement program. Utilizescomputer system as needed for information and to complete tasks, Maintains customer rights. Interacts with clients in a way that exemplifies the hospitalsmission and core values.
